agessaman f8c9decff4 fix(acl): refuse a rename or new room whose name's leftovers cannot be cleared ...

Codex confirmation round: the sweep of rows already under a name was
best-effort, so when it failed the rename or creation went ahead and the
leftovers became the room's grants. The sweep is now required: a rename
fails before the config is saved, and create_identity refuses before
adding the room.
