Does a good job in general but completely changes the url stucture of pictures for reasons unknown.

/picture?/picture-name/category/category-name
becomes
/index/category/picture/picture-name/category/category-name

I'm using Nginx with the following config: http://pastebin.com/yfWCzcK3

As you can see, the css and javascript of a picture is also not displaying good, I've heard that this is a problem with the relative path of the css and javascript, changing

$conf['data_location'] = PHPWG_ROOT_PATH.'_data/';

to

$conf['data_location'] = PHPWG_ROOT_PATH.'/_data/'; (Added slash)

Which works but causes some plugins to bug, like the upload community plugin.

Back to the url structure problem.

When you will try to open a picture it will for example go to:

http://redesign2.lolhentai.net/index/category/picture/20621-lusciousnet_lusciousnet_2014_07_06_league_of_loli_tastingx_2068449359/category/annie

Even though it normally should go to

http://redesign2.lolhentai.net/picture/20621-lusciousnet_lusciousnet_2014_07_06_league_of_loli_tastingx_2068449359

I have no idea why it is redirecting with the wrong structure.

Please open a discussion on the forum as this is supposed to work fine. Probably you nginx config does not set the PATH_INFO and/or SCRIPT_FILENAME server variables ...
