Please make picture_ext and file_ext case in-sensitive. Default setting is:

picture_ext = 'jpg','JPG','jpeg','JPEG','png','PNG','gif','GIF'
file_ext = ^^ + 'tiff', 'tif', 'mpg','zip','avi','mp3','ogg','pdf'

So big casing file_ext is missing. Even after including these, there are chance to have files with case mixed letters: Mp3, Jpg, etc. without possibility to upload them.
